We are excited to offer multiple positions for dedicated individuals to join our newly-formed team, stationed at sheltered housing schemes for older people across Newham.

As a Health and Well-being Assistant, your role will be pivotal in collaborating with older residents to elevate both their physical and mental health. Your mission will encompass reducing isolation, fostering social inclusion, encouraging physical activity, and championing good mental health awareness.

Main Responsibility

As a Health and Well-being Assistant, you will play an integral part in ensuring the health and well-being of our NHG (Newham Housing Group) residents. Your key duties will include:

  • Crafting individualized well-being plans for residents, outlining personalized objectives for their physical and mental betterment.
  • Tracking progress and preparing comprehensive reports for stakeholders, providing valuable insights into the impact of the program.
  • Implementing a diverse range of activities tailored to older residents, fostering engagement and connection.
  • Recruiting and nurturing a team of volunteers to assist in program delivery, fostering a supportive community spirit.
  • Collaborating in the preparation of quarterly monitoring meetings, contributing valuable data and insights.
  • Managing a small caseload of at-risk vulnerable residents, and promptly raising safeguard alerts when necessary.
